options for electronic submission of patient care to New York State. Two methods will qualify EMS agencies to either initiate or continue participation in the NYS Continuing Medical Education (CME) Recertification Program.<\/p><p><strong>All EMS agencies not currently documenting on an ePCR program must start the process by March 31, 2021 and finish the transition no later than June 30, 2021.<\/strong><\/p>\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-cc35fc9 elementor-widget elementor-widget-rs-heading\" data-id=\"cc35fc9\" data-element_type=\"widget\" data-widget_type=\"rs-heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t        <div class=\"rs-heading default \">\r\n        \t<div class=\"title-inner\">\r\n        \t\t      \t\t\r\n\t            <h3 class=\"title\"><span class=\"watermark\"><\/span>Enrollment in One of These Programs<\/h3>\t        <\/div>\r\n\t                <\/div>\r\n        \t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-b4b2f90 elementor-widget elementor-widget-text-editor\" data-id=\"b4b2f90\" data-element_type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\tEMS Agencies must enroll in one of these opportunities, if your EMS Agency is not already documenting electronically, by contacting their Regional Program Agency to arrange for a demonstration or to discuss in greater detail.\u00a0 When the EMS Agency makes a final decision on either the\u00a0<a href=\"\/wp-content\/uploads\/2022\/10\/DOH_5136_BLS_FR_Form_Template.pdf\" target=\"_blank\" rel=\"noopener\">BLS FR ePCR Entry Form DOH 5136<\/a>\u00a0, the\u00a0<a href=\"\/wp-content\/uploads\/2022\/10\/DOH_5136_ALS_FR_Ambulance_Form_Template.pdf\" target=\"_blank\" rel=\"noopener\">ALS First Response Form DOH 5136<\/a>\u00a0or the\u00a0<a href=\"\/wp-content\/uploads\/2022\/10\/DOH_5136_Paper_PCR_Portal_Template.pdf\" target=\"_blank\" rel=\"noopener\">Paper PCR Portal<\/a>, a completed DOH 5136 must be submitted to the Regional Program Agency for endorsement.\u00a0 The standard timeline from submission to approval should be completed within three (3) weeks for processing.\n\nThe Regional Program Agency will submit the endorsed DOH 5136 to NYS.\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-f3ebc35 elementor-alert-info elementor-widget elementor-widget-alert\" data-id=\"f3ebc35\" data-element_type=\"widget\" data-widget_type=\"alert.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<div class=\"elementor-alert\" role=\"alert\">\n\n\t\t\t\t\t\t<span class=\"elementor-alert-title\">EMS agencies encountering challenges meeting the March 31, 2021 deadline should work with their Regional EMS Program Agency and the Bureau of Emergency Medical Services to complete the transition; the Regional EMS Program Agency or the Bureau of Emergency Medical Services will assist in any way possible. This may be done with assistance from the Regional Program Agency.<\/li>\n                                                        <li>If any changes or interruptions are made to the electronic patient record system that may affect data submission, the EMS service must notify the Department, in writing, ten (10) business days in advance of implementation.
